Logging a Patient Record Request
A patient or an authorised representative may request a patient report form (PRF) from ER24. Documentation may be requested by sending an email to documents@er24.co.za.
An ER24 representative will evaluate your request and contact you to ensure the necessary consent and supporting documentation are obtained.
By law, ER24 is not permitted to disclose patient information to any party without explicit consent from the patient, their legal guardian or authorised law enforcement agencies.
